Note: An initial assessment is required to guide therapy intervention, following which ongoing treatment appointments begin. Families are welcome to request both the initial assessment AND on-going treatment appointments at the same time so that both can be booked at once where they are sure of the therapy they want. Alternatively families are welcome to request only the initial assessment before requesting on-going treatment appointments at a later date.

Complete the following to request Kid Sense schedule on-going treatment appointments for your child where they have already had an assessment (of that therapy type) in the last 12 months. You are required to provide the treating therapist with a copy of the previous written assessment report so they know what your child’s therapeutic needs are and thus how to intervene.
